Welcome to Hood Equine Veterinary Services! We are so glad to have you join our practice. We do require all clients to have a card on file and to fill out a consent to treat form to allow Dr. Hood to work on your horses. Please fill out the google form below as it will include the card authorization form, the consent form, and a code of conduct. New Clients may be required to pay a deposit prior to their appointment, which would be applied to their visit invoice.
